The Center for Neurorehabilitation Services, P.C. (CNS) is
an independent clinic located in Fort Collins,Colorado. The
mission of CNS is to provide outpatient, community based,
comprehensive neurorehabilitation and neurodevelopmental therapy
through a transdisciplinary approach to care. We provide innovative,
collaborative, supportive treatment that is focused on the
needs of the individual client resulting in increased independence
and quality of life. |

Our interdisciplinary team has provided neurorehabilitation
services since the early 1980’s. However, as with many
rehabilitation programs, CNS gradually evolved. It initially
began as a private practice in neuropsychology. In response
to community needs, it expanded to provide multiple therapeutic
interventions. In 1988, we incorporated as the Brain Injury
Recovery Program. Our initial goals included enabling brain
injury survivors and their families to re-enter their home
community of choice.

Treatment at CNS is focused around our seven major treatment
programs: Brain
Injury Recovery Program, Multiple
Sclerosis Clinic, Memory
Clinic, Adaptive
Driving Program, Headache
Management, Children's
Services, Spinal
Cord Injury, and Stroke
Rehabilitation Specialty Clinic. Therapy services are
provided by members of our transdisciplinary team that includes
neurology, neuropsychology, speech and language therapy, music
therapy, occupational therapy, physical therapy, psychological
services, social work/care coordination, and specialists in
vocational re-entry, and school re-entry.

The transdisciplinary assessment approach and the specific
therapeutic interventions provided by the different specialties
in our center were described above. Services that clients
might receive, based on diagnostic categories, are as follows:

| Physical Therapy Treatment Options |
Our Physical Therapists have many years of experience in
Neurorehabilitation. They also have strong backgrounds in
orthopedic treatment and are able to offer combined treatment
in situations that require both approaches.

| Primary Diagnoses of our Clients |
We see up to 500 new patients each year at our center, ranging
in age from about five to about 85. A physician referral is
generally required, and all treatment plans are signed as
orders. As indicated, forty percent of our new patients have
a primary diagnosis of traumatic brain injury;

While we provide a transdisiplinary approach to comprehensive
neurorehabilitation, the focus of our program is on neuropsychological
or cognitive aspects of neurobehavioral disorders. This makes
our program unique from many neurorehabilitation settings.
Following physician referral, neuropsychology is usually the
primary focus at the point of entry and provides a triage
of all symptoms presented. The assessment process may include
evaluations from any or all disciplines. In the case of direct
referral to a specific discipline, neuropsychology may not
be involved.

Clients with more severe deficits will begin therapy in the
Day Treatment Program if they meet criteria for admission.
These include potential for improvement with participation,
stable medical status, no serious behavior problems, and no
evidence of ongoing substance abuse. They must be functioning
at a minimum of Level V on the Rancho Los Amigos Cognitive
Scale. They must be able to communicate functionally and have
adequate receptive skills. They must have supervision and
support in the home environment, and a family member or significant
other must be willing to participate actively with the rehabilitation
program.

In summary, the Center for Neurorehabilitation Services,
using transdisciplinary teamwork, aims to equip each client
with the abilities and skills to live and function at the
highest possible levels. A systematic evaluation process helps
clarify and measure the client’s potential for daily
living, education, and work. The process also offers insight
into the client’s potential and needs for rehabilitation.
The Center for Neurorehabilitation Services team designs
comprehensive, individualized treatment plans based on this
detailed evaluation process. The team – in which the
client, as well as the family, are integral members –
share one goal, the client’s rehabilitation and re-entry
into the community. To accomplish our goals, we focus on developing
compensatory skills in our clients to enable them to function
to the highest level possible in their activities of daily
living.
Therapy at CNS emphasizes functional activities to ensure
the greatest amount of generalization to the real world. Unlike
many neurorehabilitation programs, we take therapy into the
location where it will be utilized, and it is not unusual
for us to do therapy in a client’s home, workplace,
or community settings such as grocery stores, shops, and public
transportation. This enables us to help the client learn strategies
to directly reduce disabilities caused by impairments, and
it helps us to remove or modify handicapping features of the
environment that could result in disability.
